Initiatives
The Center for Sustainable Health Care Quality and Equity’s Diabetes Working Group and Equity Task Force and Faith Health Alliance Advisory Council work tirelessly to eliminate health disparities and advocate for equitable health care to communities of color through the expertise and talents of professionals, advocates, and industry partners.
Diabetes Working Group and Equity Task Force
Eliminating health disparities through innovation, collaboration and data-driven initiatives.
Diabetes Working Group and Equity Task Force (DWG)
The DWG launched as a private-public partnership to promote equity in diabetes treatment and outcomes. The group includes research experts, clinicians, patient representatives, community leaders, and corporate partners who guide efforts and the continued expansion of activities in diabetes outreach through an evidence-based, sustained and expanded effort to improve health outcomes through quality improvement, education, research, advocacy and community engagement activities to address social determinants of health.

Faith Health Alliance Advocacy Council
Health care industry leaders committed to promoting sustainable health equity through partnership with Church leaders nationwide.
Through NMQF, participating healthcare industry leaders provide annual support for health activities among Faith Health Alliance members. They meet regularly with Church leaders to
discuss ongoing efforts and health equity priorities, as well as participate in their community-based events. These lead to opportunities for the healthcare industry leaders to work directly with the Faith Health Alliance members and support local communities through culturally appropriate, faith-based health ministries and the trusted voices in churches.
